If there is one thing we sure appreciate around here, it’s nice looking forms. Each Friday we plan to feature one form we’ve found around the internet wilderness, Wufoo form or not.
This week we bring you a lovely retro themed contact form by French designer Jessica Djaafar.
This form is best appreciated by [visiting the site itself](http://www.jessicadjaafar.com/). The striking retro figure used in the form is an extension of the style and other retro figures used on the site. Clicking the button (Contactez-moi) on the homepage slides this form down from the top of the screen and dims the rest of the screen, modal style.
The lovely free font [Prociono](http://www.theleagueofmoveabletype.com/fonts/13-prociono) is used as the header and button text. The button has a very nice three dimensional feel to it, emphasized by subtle and realistic rollover and pressed states. The inputs themselves also have a three dimensional feeling that is nicely balanced by the button (they button feels as tall as the inputs are deep).
A link in the lower left hides the form and reveals other contact information, which is a nice place to tuck such information away. It’s like saying: please use my form first, unless you really need to use one of these other contact methods.
